Purpose of the App

WellnessMAP is a health tracker and education platform. It is for educational and informational purposes only. The app displays self-reported user experiences for informational purposes only.

The app does not diagnose, treat, cure, prevent, or recommend interventions for any condition. Nothing presented should be interpreted as medical advice.

Voluntary Participation

Your participation is entirely voluntary. You are free to stop at any time. You are not required to complete the assessment or any part of the app. You will not be paid. This app is not part of any medical care or treatment.

1. Information We Collect

We collect information necessary to provide and operate our services:

• Account Information: Your email address (required for sign-in) and name (if you choose to provide one).
• Health Survey Responses: Your answers to the intake questionnaire, including any free-text or voice responses you provide.
• Chat Messages: Your conversations with the health education assistant, including any images you attach.
• Health Tracking Data: Daily tracking entries you log for the lifestyle areas you are exploring.
• Session Data: IP address and device information are recorded when you sign in for security and session management.
• Usage Analytics: We use PostHog to collect anonymous usage data such as screen views, app version, and device type. This data does not identify you personally.

2. Information We Do Not Collect

• We do not collect your precise location (GPS) or geolocation data.
• We do not collect advertising identifiers or track you across other apps.
• We do not connect to, read from, or write to Apple HealthKit, Google Health Connect, or any other third-party health platform.

3. How Your Data Is Used

• Your survey and chat data are processed by AI services (hosted on Microsoft Azure) to generate educational health insights.
• If you upload food images in chat, they may be analyzed by a third-party nutrition service (Passio) to provide nutritional information.
• Your data is not sold to third parties, advertising networks, or data brokers under any circumstances.
• Your data is not shared with insurance companies.

4. Infrastructure Providers

We use trusted cloud service providers strictly for hosting and infrastructure:

• Microsoft Azure: AI processing and cloud infrastructure
• Cloudflare: Security and content delivery

These providers process data on our behalf and cannot access or use your data for their own purposes. We may disclose information only if strictly required by a valid legal subpoena or court order.

5. Your Rights and Data Deletion

You own your data and maintain the right to delete it at any time.

• In-App Deletion: You can delete your survey responses, chat history, health tracking data, or your entire account through the app's Profile settings.
• Email Deletion Request: You may also request complete data deletion by emailing privacy@wellnessmap.ai with the subject line "Account Deletion Request." We will process these requests within 14 days.

Deleting your account permanently removes all of your data from our systems and cannot be recovered.

6. Data Storage and Processing

Your data is stored and processed securely in the United States, governed by the laws of the State of Florida.

7. Age and Eligibility

WellnessMAP is designed for adults who are at least 18 years of age and reside in the United States. By using the app, you represent and warrant that you meet these requirements. If we become aware that a user does not meet these requirements, we will take immediate steps to delete their information.

8. Disclaimers

WellnessMAP is a health tracker and education platform. We are not a medical provider.

The data shown in the app are observational and were not collected under controlled or experimental conditions. No cause-and-effect relationships can be established. Reported experiences may be influenced by reporting bias, self-selection, or multiple unrelated factors.

Groupings such as "users in this group" are based on shared reported characteristics and are descriptive only. They do not predict outcomes for any individual.

Any mention of lifestyle areas alongside reported experiences does not mean that any food, supplement, or lifestyle factor is effective for any condition.

Statements presented in this app have not been evaluated by the Food and Drug Administration.

The information and insights provided by our platform are for educational and informational purposes only and should not replace professional medical advice, diagnosis, or treatment. Always consult your healthcare provider before making changes to your health routine.

9. Risks and Benefits

Risks: Some people may feel discomfort or confusion when viewing health-related content. If the app detects high-risk content, it will interrupt and display emergency contact information.

Benefits: Some users may find it interesting or helpful to see what others with similar concerns reported. This is not guaranteed, and no clinical benefit is implied.
